Well I didn't find this, a friend sent it to me through the email, but it seems to be the membership form for the Society of the Ancients. And it seems that this is the membership form:

Now I am pretty new this so if its a fake or something, someone tell me and I'll keep on searching for something else.
EDIT: It seems that the e-mail hasn't been activated yet... Like the sota.com website so...

Like I've said before. Just posting "old" isn't helpful
